A routine visit is conducted every 30 days as required by state and federal regulations.  This includes a clinical and regulartory review of each resident's medication regimen, aspects of medication storage, and assessment of medication use which together sucessfully derive a comprehensive overview of your facilities compliance with Pharmacy regulations in long-term care.   A comprehensive Pharmacy Consultation Report is provided at the end of the visit .  This report contains both nursing and physician recommendations which address ways to enhance the pharmacy component of care for your residents and increase continuity of care.

Provide analysis of current pharmacy consultant services to identify areas which need adequate focus for a more successful survey

Intervention during state and federal survey to insure deficiencies are prevented or corrected promptly

Medication Regimen Review (MRR)

Audit medication storage and availability

Drug Destruction

Medication Pass observation and training, including in-services for problems identified

Presentations/In-services for staff and family nights

Attend QA or other facility meetings which request pharmacist input/information to assess and insure quality of medications towards positive therapeutic outcomes for residents.

Work with physicians to increase quality of resident care by insuring proper use and monitoring of medication, and adjustments in regimen to achieve maximum therapeutic efficacy.

Insure facility complies with all pharmacy related state and federal regulations

We will also assist with drug return programs with participating pharmacies to reduce facility cost of Medicare A residents.
